Special Considerations for Renal Impairment

Reduce Cephalexin dosage for patients with impaired kidney function. Creatinine clearance (CrCl) is key to dose adjustment.

For CrCl 30-50 mL/min, reduce the dose by approximately 50%. For CrCl less than 30 mL/min, a further dose reduction or even discontinuation may be necessary. Always consult a nephrologist for patients with severe renal impairment.

Hemodialysis removes Cephalexin, so consider administering a supplemental dose after dialysis, dependent on the individual’s CrCl and dialysis clearance.

Closely monitor patients for signs of toxicity, such as nausea, vomiting, diarrhea, or changes in urine output. Regularly assess renal function via serum creatinine levels.

Dosage adjustments depend on individual patient factors and should always be determined in consultation with medical professionals, considering the severity of renal impairment and other co-existing medical conditions.
