Fluoroquinolones offer a powerful alternative to amoxicillin, particularly against gram-negative bacteria. These bacteria possess an outer membrane making them resistant to many antibiotics, including penicillin-based drugs like amoxicillin. Ciprofloxacin and levofloxacin are common examples, effectively inhibiting bacterial DNA replication.

Mechanism of Action

Fluoroquinolones target two enzymes crucial for bacterial DNA replication: topoisomerase II (DNA gyrase) and topoisomerase IV. By blocking these enzymes, fluoroquinolones prevent bacterial DNA replication and repair, leading to bacterial cell death. This mechanism differs significantly from amoxicillin’s, which inhibits bacterial cell wall synthesis.

Spectrum of Activity

Their broad spectrum includes Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, and Haemophilus influenzae–common culprits in urinary tract infections, respiratory infections, and other serious illnesses. However, resistance is a growing concern, so appropriate testing is necessary before prescription.

Considerations for Use

While effective, fluoroquinolones carry potential side effects like tendonitis and peripheral neuropathy. Therefore, they are generally reserved for infections where other antibiotics have failed or are unsuitable. Doctors carefully weigh benefits against risks before prescribing these drugs.
