Understanding the Differences Between Metoprolol and Bisoprolol

Metoprolol and bisoprolol are both beta-blockers, but they possess key distinctions impacting their use. Metoprolol exists in two forms: metoprolol succinate (extended-release) and metoprolol tartrate (immediate-release). Bisoprolol, conversely, is only available in an extended-release formulation. This directly affects how often you take the medication and the duration of its effects. Metoprolol tartrate needs to be taken twice daily, while metoprolol succinate and bisoprolol are usually once-daily medications.

Selectivity: Metoprolol is a relatively non-selective beta-blocker, meaning it affects both beta-1 and beta-2 receptors. Beta-1 receptors primarily reside in the heart, while beta-2 receptors are found in the lungs and blood vessels. Bisoprolol demonstrates greater beta-1 selectivity, minimizing effects on beta-2 receptors. This difference can be significant for individuals with asthma or chronic obstructive pulmonary disease (COPD), as non-selective beta-blockers can worsen respiratory symptoms by constricting airways. Bisoprolol poses a lower risk in this regard.

Intrinsic sympathomimetic activity (ISA): Metoprolol lacks significant ISA, while bisoprolol possesses minimal ISA. ISA refers to a drug’s ability to partially stimulate beta-receptors. This can be beneficial in mitigating some of the negative side effects associated with beta-blockers, such as fatigue and bradycardia. The practical difference, however, is often minimal, and both drugs can cause similar side effects.

Lipid Effects: While both drugs generally don’t raise cholesterol, some studies suggest metoprolol succinate may have a more neutral effect on triglyceride levels compared to bisoprolol. Always discuss lipid profiles with your healthcare provider.

Conversion: Direct conversion from metoprolol to bisoprolol requires careful monitoring by your physician. Dosage adjustments will be necessary based on individual responses and medical history. They will carefully consider the specific type of metoprolol you are currently using (tartrate or succinate) before determining the appropriate bisoprolol dose.
