Closely monitor vital signs, including blood pressure, heart rate, and respiratory rate, at least every 15 minutes initially, then adjust frequency based on patient response. Observe for signs of dehydration, such as decreased urine output, dry mucous membranes, and hypotension.
Fluid balance: Accurately record fluid intake and output. Calculate urine output hourly; report oliguria (low urine output) immediately. Assess hydration status through skin turgor and capillary refill time. Maintain intravenous access for fluid resuscitation as needed.
Electrolyte monitoring: Regularly check serum potassium, sodium, chloride, and magnesium levels. Hypokalemia is a common complication of Lasix use; initiate potassium replacement therapy if indicated by lab results. Closely monitor for signs and symptoms of electrolyte imbalances, such as muscle weakness, arrhythmias, and confusion.
Renal function: Monitor serum creatinine and blood urea nitrogen (BUN) levels to assess kidney function. Report any significant changes to the physician.
Weight monitoring: Daily weight measurements help assess fluid status. Significant weight loss indicates fluid depletion, necessitating prompt intervention.
Allergic reactions: Although rare after transfusion, continuously assess for signs of allergic reaction to the transfused blood product. These may include rash, itching, hives, and respiratory distress.
Document all observations, interventions, and responses diligently. Maintain clear and concise communication with the healthcare team regarding patient status and any concerning trends.
