Interpreting Digoxin Levels: Therapeutic vs. Toxic

Generally, a therapeutic digoxin level ranges from 0.8 to 2.0 ng/mL. However, individual responses vary significantly. Levels below 0.5 ng/mL suggest inadequate treatment, potentially necessitating dosage adjustments.

Levels above 2.0 ng/mL increase the risk of toxicity. Symptoms of digoxin toxicity include nausea, vomiting, loss of appetite, irregular heartbeat (arrhythmias), and visual disturbances (blurred vision, halos around lights).

Interpreting results requires clinical judgment. Consider the patient’s age, kidney function, and other medications. A seemingly high level might be well-tolerated in some individuals, while a slightly lower level might cause problems in others.

Regular monitoring is key. Frequent blood tests, particularly during initiation of treatment and dosage adjustments, ensure safety and efficacy. Adjustments should be made cautiously based on both the digoxin level and the patient’s clinical response.
