Suspect Lasix-induced acute kidney injury (AKI) when a patient develops oliguria or anuria shortly after starting Lasix, especially if they have pre-existing renal impairment or dehydration.
Laboratory tests are key. Measure serum creatinine and blood urea nitrogen (BUN) levels. A significant rise in these values, coupled with reduced urine output, strongly suggests AKI. Monitor serum electrolytes, particularly potassium, which can rise dangerously with decreased renal function.
Urine analysis provides additional information. Assess urine output, specific gravity, and the presence of sediment or casts. Examine for features of prerenal azotemia (e. g., high urine osmolality and sodium concentration) which may point towards volume depletion, a potential underlying cause for Lasix-induced AKI. Contrast this with intrinsic renal failure patterns (e. g., low urine osmolality and low urine sodium).
Imaging may be helpful. Ultrasound can assess kidney size and identify any structural abnormalities contributing to AKI. However, this often isn’t needed to diagnose Lasix-associated AKI. The clinical picture and laboratory values typically provide sufficient information.
Consider alternative diagnoses. Rule out other causes of AKI, such as nephrotoxic medications, pre-existing kidney disease, or obstructive uropathy, using clinical history and further investigations as needed. This ensures a complete picture.
Timely intervention is crucial. Early diagnosis allows for prompt withdrawal of Lasix and implementation of supportive measures, potentially mitigating AKI severity. Careful hydration and monitoring of electrolyte imbalances are fundamental aspects of management.
