Confirm the diagnosis with physical examination, noting the characteristic bluish-gray discoloration, typically affecting sun-exposed areas. Biopsy is rarely needed but can help rule out other conditions.

Differentiating doxycycline pigmentation from other causes of hyperpigmentation requires careful consideration. Minocycline pigmentation, for example, presents similarly but often appears more brown or gray-brown than the bluish hue of doxycycline pigmentation. It also often involves areas not typically exposed to sun.

Other conditions to consider include: amalgam tattooing (metallic mercury deposition from dental fillings), post-inflammatory hyperpigmentation (following skin trauma or acne), and drug-induced hyperpigmentation from other medications like amiodarone or antimalarials. Careful evaluation of the patient’s medication history and a detailed clinical picture are critical in arriving at the correct diagnosis. A Wood’s lamp examination may help in some instances. The location, color, and pattern of the pigmentation help distinguish doxycycline pigmentation from similar conditions.
